(The State of AI in Clinical Trials)’ 보고서에 따르면, 글로벌 임상 연구 업계는 이제 AI를 단순 효율화 도구가 아니라 임상 운영 경쟁력을 좌우하는 핵심 인프라로 보기 시작했다.

World Clinical Trials Day, “Speed of AI Adoption Divided Clinical Outcomes”… Changes in Clinical Competitiveness Shown in Medidata Report
May 20, 1747. James Lind, a British naval surgeon, conducted an experiment to compare different treatments on sailors suffering from scurvy during voyages. He provided some sailors with citrus fruits, such as limes and oranges, and observed a noticeable improvement in the symptoms of those who consumed them. This experiment, referred to today as the "world's first modern clinical trial," is considered the starting point of modern clinical trials.
To commemorate this, the European Clinical Trials Infrastructure Network (ECRIN) designated May 20, 2005, as 'World Clinical Trials Day.' Since then, major countries in the United States, Europe, and Asia have been highlighting the advancement of clinical trials and the contributions of researchers around this day every year. Korea has also been holding related events since 2017, led by the Ministry of Health and Welfare and the Korea National Clinical Trials Support Foundation (KoNECT).
And now, with the 2026 World Clinical Trials Day approaching, the topic the industry is focusing on most is undoubtedly artificial intelligence (AI). Analysis suggests that the clinical trials industry's interest is rapidly shifting from "whether to adopt AI" to "how to translate AI into actual results."
According to the 'The State of AI in Clinical Trials' report recently released by clinical trial solutions company Medidata, the global clinical research industry has now begun to view AI not merely as a tool for efficiency, but as a core infrastructure that determines the competitiveness of clinical operations.
Medidata '2026 Status of AI in Clinical Trials' Report
The era of “Should we adopt AI?” is over… The era of “How to expand?”
This report contains the results of a survey conducted jointly with the Everest Group targeting 200 clinical operations managers from pharmaceutical companies, biotech firms, and CROs (Clinical Research Organizations) in North America and Europe.
According to the survey results, 92% of respondents said they plan to expand their investment in AI within the next one to two years. Additionally, 82% responded that they expect a return on investment (ROI) of two to three times through the adoption of AI.
In particular, the atmosphere in the industry has changed significantly compared to last year. While the industry's interest remained in limited areas such as data collection and quality control until 2025, this year the scope of AI utilization is expanding to include data interpretation and analysis, operational automation, and the improvement of efficiency across clinical processes. In a report, Medidata assessed that “the industry’s question has now shifted from ‘whether to use AI’ to ‘how to responsibly scale AI.’”
However, the entire industry has not yet fully transitioned to an AI-centric model. In fact, only about 33% of companies had expanded AI to the stage of enterprise-wide implementation and deployment. 82% of respondents stated that they had been using AI in clinical operations for 18 months or less.
The use of AI was yielding rapid results, particularly in repetitive and rule-based tasks. 46.5% of respondents reported experiencing improvements beyond expectations in areas such as task automation, data cleaning, and query resolution. On the other hand, key performance indicators (KPIs), such as shortening the overall duration of clinical trials or reducing protocol dropouts, were not yet clearly evident across all companies.
However, the performance of early adopters was distinctly different. The group of companies utilizing AI for over 18 months recorded performance approximately twice as high as the overall average in shortening clinical trial periods. They also demonstrated greater improvements than the overall respondent group in reducing protocol dropouts and automating workflows.
Medidata analyzed this, stating, “The value of AI is not short-term, and cumulative effects begin to appear starting with companies that have completed their first implementation cycle.” Indeed, the report warned that “companies delaying the adoption of AI now may experience a structural gap in clinical competitiveness in the future.”
AI Spreading to the Entire Clinical Trial Process… “Data and Governance Are Key”
The scope of AI utilization is also expanding across the entire clinical trial lifecycle.
In the early stages, AI is utilized for protocol design, patient recruitment, and site selection. Medidata explained that there is a growing number of cases where AI-based modeling is used to analyze historical clinical data to predict suitable patients or simulate the likelihood of clinical success.
Medidata AI Supporting the Entire Clinical Trial Lifecycle
In the actual operational phase, AI is being applied to data integration and quality management, query automation, and Risk-Based Quality Management (RBQM). In particular, AI-based data quality management tools analyze vast amounts of clinical data in real time to automatically detect anomalies and data inconsistencies.
The use of AI is also expanding at the regulatory submission stage. According to the report, 69.5% of respondents are already using AI in the data integration and standardization process, and the application of AI is increasing in the automation of Clinical Research Reports (CSR) and the review of regulatory documents.
The industry is paying particular attention to 'Agentic AI' and digital twin technology. According to the report, 31% of respondents predicted that AI would be used for protocol simulation in the future, while 26.5% predicted that digital twins, which virtually model patients, testing sites, and clinical results, would become widespread within two to three years.
However, the challenges are also clear. The biggest obstacles identified by the industry were integration complexity (79.5%), model accuracy (77.5%), and data discontinuity (75%). Medidata analyzed that "the success or failure of AI depends more on data quality and governance frameworks than on the latest models."
The actual report presented governance frameworks, such as legal and compliance reviews, Human-in-the-loop monitoring, data traceability, and access control, as key conditions for AI utilization.
Hyeji Kim, Senior Vice President and Head of Marketing for Medidata Asia Pacific, stated, “The use of AI in clinical trials is no longer a matter of simply adopting it, but rather how to integrate it into operational workflows,” adding, “The performance gap between companies that proactively adopt AI and those that do not is highly likely to widen further in the future.”
Medidata AI
