Struggling to Walk? Leg Pain May Not Just Be Aging, but 'Foraminal Stenosis'

-Narrowing of the nerve passageways threatens the quality of life for middle-aged and elderly patients.

-Symptoms include relief when bending forward, worsening pain when standing straight, and nocturnal calf cramps.

-From conservative management to cutting-edge biportal endoscopic surgery, accurate diagnosis remains the key to treatment.

SEOUL

— For Mr. Kim (67), a former office worker who spent his weekends hiking, a sudden decline in activity has become a source of deep anxiety. The culprit is a recurring, agonizing pain and tension in his legs that flares up after only a short walk. Initially, Kim dismissed it as a natural loss of stamina due to aging. However, as the pain intensified, he found himself unable to walk for more than ten minutes without sitting down to rest by the roadside. Upon visiting a hospital, Kim was diagnosed with

Foraminal Stenosis

.

"When people experience back pain and leg numbness, they often immediately think of a herniated disc," says Dr. Park Jin-gyu, Director of PMC Park Hospital and President of the Korean Neurosurgical Hospital Association. "However, if you experience 'intermittent claudication'—a pattern of walking and stopping repeatedly due to pain—it is highly likely to be a form of spinal stenosis known as Foraminal Stenosis."

The Narrowing Gateway of the Spine

The human spine serves as the body’s central pillar. Between the vertebrae, there are specialized openings called

foramina

, which act as tunnels for nerves to exit the spinal cord and travel down to the legs. Foraminal stenosis occurs when these tunnels narrow, compressing the nerves and triggering pain alongside various neurological symptoms.

While symptoms vary based on the location of the compression, the most common signs are leg pain and numbness. Patients often describe a tingling or "pulling" sensation that radiates from the buttocks down through the thighs, calves, and toes. Common descriptors include a sensation that the legs are "about to burst" or feel "like someone else's skin."

The 'Shopping Cart' Relief: A Telling Sign

A hallmark of this condition is how pain fluctuates with posture. Patients typically find temporary relief when leaning forward, as this posture physically widens the foraminal space. This explains why many elderly individuals lean on strollers, walkers, or shopping carts while walking. Conversely, standing upright or bending backward further constricts the foramina, exacerbating nerve compression. Furthermore, impaired blood flow often leads to painful nocturnal calf cramps that disrupt sleep.

Causes: Beyond Mere Aging

The primary driver of foraminal stenosis is "degenerative change"—the natural wear and tear of aging. As the years pass, the bones and ligaments supporting the spine thicken and roughen. These enlarged tissues eventually encroach upon the internal space of the foramina.

However, aging is not the only factor. A spinal disc may bulge backward to block the path, or the loss of disc height (dehydration) can cause the space between vertebrae to collapse, effectively "crushing" the foramen. Additionally, "bone spurs" (osteophytes)—sharp bony outgrowths—can directly irritate the nerves, causing chronic inflammation.

From Physical Therapy to Endoscopic Innovation

Because the symptoms are complex and the causes varied, a precise diagnosis by a specialist is essential. "The key to treatment is identifying exactly which structure is compressing which nerve and to what extent through MRI and other detailed scans," Dr. Park explains.

Treatment is approached in stages:

Early Stage:

If symptoms are mild, non-surgical "conservative" treatments are often sufficient. This includes medication, physical therapy, and exercise regimens designed to strengthen core muscles and increase flexibility.

Middle Stage:

For more persistent pain,

Nerve Block

(injection therapy) can be an effective alternative, injecting medication directly near the nerve to reduce inflammation and swelling.

Advanced Stage:

If conservative measures fail, or if sensory loss and paralysis occur, surgical intervention is considered.

Modern medicine has shifted from large-incision surgeries to

Minimally Invasive Surgery (MIS)

. Two leading methods include

Microsurgical Foraminal Decompression

, which uses high-powered microscopes to precisely remove thickened ligaments or bone spurs, and

Biportal Endoscopic Spinal Surgery

. The latter involves inserting an endoscope and surgical tools through two small 5mm incisions. Due to minimal tissue damage, these procedures offer less pain, reduced bleeding, and faster recovery times, making them suitable for elderly patients or those with underlying health conditions.

"In cases where spinal instability is severe or the stenosis is extensive, spinal fusion—fixing the vertebrae with screws—may be necessary," warns Dr. Park. "As with all conditions, seeking medical attention at the first sign of pain is the shortcut to shorter treatment times and a healthy later life."

While foraminal stenosis is a daunting threat to mobility in old age, it is a manageable condition. If your stride isn't what it used to be, do not dismiss it as a mere byproduct of age; proactive medical intervention can restore the freedom to walk.
